Prize Draw Summary (Key Information)

  • As a thank you for completing this survey, you can choose to enter a prize draw to win a £25 ‘Love 2 Shop’ voucher. Only survey respondents who actively choose to enter the draw will be entered, there is no automatic entry.
  • This is a free give away, and does not constitute a lottery.
  • The winner will be selected at random
  • Your contact details are collected only to administer the prize draw and stored separately from your survey responses. This is in line with our privacy policy which you can find here: https://www.healthwatchliverpool.co.uk/privacy.
  • Open to Liverpool residents aged 13+.
  • One entry per person.
  • The winner will be chosen at random from all valid entries received by Friday 14th August 2026
  • The winner will be contacted by email to confirm, within 5 working days of the draw closing. The prize will then be sent by email (please provide details on entry).
  • The prize can’t be exchanged or transferred, and no cash alternative is available.

Prize Draw Full Terms and Conditions

1. Who we are

This prize draw is run by Healthwatch Liverpool, [which is delivered by LARIDAE CIC], based at 151 Dale Street, Liverpool, L2 2AH (“we”, “us”, or “the Organiser”).

2. Who can take part

  • The draw is open to anyone in Liverpool aged 13 or over who completes the ‘Young People and Sexual Health’ survey
  • Consent to take part in this prize draw and share your contact details, can only be given by people who are 13+. If you are not 13+, please do not complete this form/enter the prize draw. By entering this prize draw, you are confirming that you are over 13 years old.

3. What you can win

  • There is one prize: a £25 [‘Love2Shop’] voucher
  • The prize can’t be swapped for cash, transferred, or exchanged for anything else.
  • Once the prize has been sent, we’re not responsible if it’s lost, deleted, stolen, or expires.
  • The voucher is subject to Love2Shop.co.uk’s own terms and conditions.

4. How to enter and how the winner is chosen

  • Upon completion of our survey, respondents will be provided with a link to a separate online form, where they can choose to enter the prize draw by submitting their contact information.
  • In order to run this prize draw, we require some personal information about you, so that we can contact the winner. 
  • Only one entry per person is allowed.
  • We will use a fair random process that can be evidenced, to draw the winner, from all valid entries received by the closing date Friday 14th August 2026, 5pm.

5. How the prize will be sent

  • We’ll contact the winner via the email provided on the prize draw entry form, within 5 working days of the draw, to verify the email address. 
  • The voucher will be sent by email within 5 days once the winner has been confirmed and their email address verified.
  • If we can’t reach the winner within 14 days, we may choose another winner at random.
  • Please make sure your contact details are correct — we can’t take responsibility if we cannot reach a winner because of wrong or incomplete information.

6. How we use your data

  • The lawful basis for processing personal data for this prize draw, is consent.
  • We’ll use your personal information only to run this prize draw, and not for any other purposes
  • We will only keep your contact details for as long as is required to successfully complete the prize draw and send the prize to the winner, and we will then delete your details.
  • If you do not consent to us collecting your data and contacting you, do not complete this form with your details.
  • You can withdraw your consent and tell us to delete your entry, at any time, by emailing us at engagement@healthwatchliverpool.co.uk, or calling us on 0300 77 77 007. 
  • We’ll look after your data in line with the UK GDPR and the Data Protection Act 2018.

8. Our responsibility

We’re not responsible for:

  • Technical problems that stop you entering;
  • Any issues with the voucher itself, after we’ve sent it; such as problems caused by the voucher supplier (Love2Shop.co.uk).

While consideration has been given to ensuring the voucher prize is an appropriate choice for prize draw entrants, we are not responsible for the choice of what the winner chooses to spend their voucher on, and any risk or harm associated with the voucher-purchased goods. 

9. Other important information

  • We may need to change, pause, or cancel the draw if something unexpected happens.
  • This is a free give away, and does not constitute a lottery.
  • Our decision on any matter relating to the draw is final.
  • These Terms are governed by the laws of England and Wales, and any disputes will be settled in those courts.
